Helping NJ Family Care members experiencing housing insecurity find and maintain safe and affordable housing
Homeless Solutions’ newly launched Housing Supports Program aims to connect NJFamilyCare (Medicaid) members with safe and affordable housing through intensive Housing Case Management and Navigation services.
Approved under Federal 1115 waiver and launched on July 1, NJ Family Care Housing Supports program allows Medicaid to fund pre-tenancy and tenancy sustaining services for eligible members experiencing homelessness or housing instability who meet qualifying clinical criteria. Licensed Medicaid providers, like Homeless Solutions, Inc., then deliver these services that help people find, move into, and successfully remain in safe, affordable homes. This has been a collaborative effort between HSI, the NJ Department of Community Affairs (DCA), NJ Division of Medical Assistance and Health Services (DMAHS) and managed care partners to turn policy into impact.
Making an Impact
In August 2025, HSI received its first Housing Supports referral for a 73-year old Medicaid member with chronic health conditions and mobility challenges. He had previously been supported by HSI programs and urgently needed to be relocated from his current home due to a foreclosure on his apartment building and unsafe conditions.
HSI accepted the referral while still in the process of building its Medicaid program and immediately coordinated rapid outreach, an accessibility-focused housing search, and documentation support.
Within 60 days, HSI and partners secured a senior apartment and worked to help get the member relocated as quickly as possible. He signed his lease on October 1. In just days, staff observed meaningful changes. “This member had previously been sleeping in a chair. Having a bed to lie in at night has greatly improved his comfort and mobility. Referral Process
Eligible households must be referred by an agency or a managed care organization. Referring entities must complete an HSI Housing Supports Referral Package with the NJ Family Care member and forward to Homeless Solutions for processing.
